Authorization Agreement

I hereby certify, represent, and warrant that the information supplied in this application by me are true and correct. I agree to a full investigation of all information by Zelly Salon Suites, its representatives, or assigns (collectively “Zelly Salon Suites”) to verify and I authorize Zelly Salon Suites to conduct such additional credit checks, background checks, and criminal investigations as deemed necessary and to the fullest extent permitted by law. I agree that this application is and shall remain the property of Zelly Salon Suites.

By my signature below, I authorize all corporations, former employers, credit agencies, educational institutions, law enforcement agencies, city, state, county and federal courts and agencies, military services and persons and entities to release all information they may have about me, including criminal and driving history. This authorization shall be valid in original or copy form.

I further understand that submission of this information does not obligate either of the parties to enter into any commercial relationship or business transaction in any manner.

If my application for tenancy is approved, I may, from time to time, elect to provide certain content in various media (including, but not limited to, photographs, digital images, video clips and audio clips), to Zelly Salon Suites that includes images of me, my studio space design, or my personal branding (“My Content”). Zelly Salon Suites may also, from time to time, as part of its own marketing initiatives, create or otherwise utilize certain content in various media that includes images of me, my studio space design, or my personal branding (“IS Content”). By signing below, I hereby expressly consent to Zelly Salon Suites using My Content and IS Content. This consent is perpetual, royalty free and worldwide in scope, but may be revoked by me with written notice, sent U.S. certified mail, to Zelly Salon Suites specifying the content for which consent is withdrawn. Upon receiving such notice, Zelly Salon Suites will diligently act to remove the content. However, I acknowledge my consent is not required for IS Content pertaining to Zelly Salon Suites business in general, where I, my studio space design, or my personal branding are only incidentally featured. I also warrant and represent that My Content will not infringe any third party’s rights and I agree to indemnify and hold harmless Zelly Salon Suites from all liability for any third-party claims of infringement relating to My Content.
